In addition to be able to add a reason field and a comment field you can add any fields so long as it is in the specified User Table.
Select the user table from the Table drop down list.
Table - Shows a list of user tables from which fields can be added to the Request screen for the specific Request Type.
In order for a table to show up as an option in the Table field, the table must be a User Table with the Table Type of Single Record, Level of Other, and Parent Table of Request. See User Tables for additional information on constructing user tables.
Only one table may be used per Request Type and the table is locked once the Request Type is saved.
When a table is selected, the Available Fields are displayed. Fields can be added from the Available Fields section to the Selected Fields section by highlighting one or many field names in the Available Fields list and click
. Fields can be removed from the Selected Fields section by highlighting one or many fields in the Selected Fields list and click

Available Fields - Displays all fields associated with the selected Table that can be added to the Request screen for the custom Request Type. It is important to note that fields configured with a field type of: Rich Text, Account, Vendor, Hierarchical Lookup, or Currency will NOT be listed as available. Supported field types will display appropriately (e.g. Lookup fields will appear as drop down lists, money fields will appear as amounts plus currency).
Selected Fields - Displays all fields chosen to be displayed on the Request screen for the custom Request Type.
The order in which the fields display on the Request page can be modified by highlighting one or many fields in the Selected Fields section and using
and
to change the field placement order.
Once configuration of the Request Type is complete, clicking [Save] will save the Request Type. Request types can be edited by clicking next to the Request Type that is to be edited. Request Types can be deleted by clicking [Delete]. Please note that, while most options including available fields and field order can always be edited, if a Request Type has been used for an assignment request, that Request Type will no longer be able to be deleted. It can however be marked as inactive.
By default, all fields added to a request are not required fields however specific fields can be made required by using the Data Dictionary Manual Override feature to set the desired required fields when viewing the desired Request Type on the Requests page.
